I an plate8!(around 165-170 pretty hard for reps. I currently weigh about 182.

stop going so heavy. 1-2 rep maxes don't build muscle. All you're doing is frying your nervous system.

Yea youre not doing that much weight so not too big of a risk but always get a spotter once you start doing heavy benches man.

Its 3x5, I stalled on the final rep of my second set of 5 for 170. This was my second stall, so Ill be resetting back to 155 (90%)

Currently I do about 3 minutes on heavy compounds. Benching doesnt particularly exhaust me. (Unlike deadlifts)

Ive been having issues with my squats lately too. If push above 255 I seem to have trouble comfortably going going to parallel, but Ive never failed a squat yet.

not sure how you warm up but a problem I used to have was doing too many warm up sets before my actual working sets. And by the time I was on my second set I was already tiring out. Just something to keep in mind.

Usually two warmup sets for bench.

3x5 @ 85
3x5 @ 135

Generally I dont fail because I am pretty good at telling when I am going to stall a rep.
