Set it to a difficulty where everything takes x4 damage, including you, and install some mods that increase enemy spawns.

Difficulty should come from better AI not just the enemy taking more damage.

Immersive + Deadly Wenches fixed this for me. On one hand, the enemies have greater health, but their attack power is more measured over comparable vanilla enemies, which generally results in more balanced combat. On the other hand, their special powers are ridiculously OP; while this can be frustrating at times, it does force you to fall back and flee / go on the defensive until their powers wear off. As long as you don't have a problem with the game world being overpopulated with attractive females, I'd recommend it.
