[Preferably games that are hard because of deliberate design choices & not because of glitches/poorly-thought-out obstacles/etc.]

Kind of a cliche choice, but probably the NES version of Ghosts 'n Goblins. As far as games I actually own/have put at least an hour into, R-Type for the Turbografx-16 is pretty brutal.

From a pure playing standpoint I have a lot of trouble with difficult arcade games, since I just don't really have any familiarity with the control stick setup. Even a game like regular old Ms. Pacman on arcade is difficult for me since you control everything by joystick and it gets speedy fast.

I forget the name. I played it with friends as a little kid. You're like a sheriff going through the streets of some western town until you get to the boss, who's a huge Mexican sombrero guy. The thing is he has like 9000 health. We spent hours fighting him and we played in shifts. It's one of my fondest gaming memories though.
