They feel personally wronged by you for whatever reason, say maybe they think you fucked their girlfriend or something, so they're trying to hurt you. The only guarantee you get is that you won't die but anything else is possible. Nobody will know you got the money unless you want them to but the whole thing will be recorded by someone and put on YouTube. The $1m will be after any medical expenses are covered, which will come at no cost to you.
