I think the joke is either that you're not able to create a character this attractive in vanilla Fallout 4 because Bethesda character models pretty much always look bad.
Or "No Cousin Stuff" being a joke about all the adult mods available for Skyrim/Fallout 4.

And the reply "would it really be fallout without mods tho?" is referring to the fact that literally nobody plays vanilla Skyrim/Fallout on PC. Why would you when there's hundreds to thousands of mods, many of which vastly improve the games' mechanics and looks along with fixing all the bugs Bethesda refuses to fix even after multiple rereleases.
