Probably is actually possible, I did something roughly similar one time I took too many tabs.

Was messing with my thought processes and managed to get two concurrent senses of self that were generating ideas independently and that I could still consciously track.

Freaked me out, and I figured that couldnt lead anywhere except a chemically fueled mental break so I shut that shit down fast.

Idk, the fact that it's host-forced, is generally not resulting in negative consequences from just the practice itself (with obvious social stigmas not counting as that is a negative as a result of society rather than the actual action), and that tulpas generally do go away if the host chooses to abandon them makes it not really something I think would be classified as a mental illness.
