Redirecting a card to be banished (AKA what Shifter, Cosmos, D-Fissure, etc. do) doesn't count as banishing by a card effect, so a "send to the GY" effect like Dingirsu would indeed get around Gymir's protection from being banished even if an effect like Shifter being active would cause it to be banished instead. God, this game really is for lawyers.
Yes card effect almost always refers to something that happens during a chain resolution. Continuous or lingering effects do not cause a new chain, though some continuous effects do, but they're actually continuous effects that cause an ignition effect rather than just being only a continuous effect, if that makes sense.

I'm trying hard to think of a potential counter example, but I'm fairly sure that's how it works
