Mystical Ninja Starring Goemon - I don't understand what's going on here or why but I love every aspect of it. Visually it's hilarious, while still being clean. The music it great, the font is goofy but in a way that fits, and something about the usual "Continue" and "Quit to Main Menu" being replaced by "Try again" and "Good bye" just kills me

Zelda II: The Adventure of Link - I'll fully admit I've never been a Zelda guy, but there's a reason this is as infamous as it is. It's terrifying, and possibly stroke inducing. And the sound design here sells the whole thing.

Final Fight - Now this is how you do a Continue screen. Love it for the same reasons as Ninja Gaiden, but it's unfortunately missing the "ohhhhh"

Diablo - Pretty simple, but oddly inspiring! It's not mocking you or displaying cruelty, it's just an inspirational message that makes it feel like what you've done mattered, despite your loss.

Metroid Prime - I really like what they're going for here, but man that life support ending sound would get irritating quick.

Mid Tier
Super Mario Bros. - The death music is probably the most iconic there is. The Game Over screen itself is as basic as they come.

Chrono Cross - Aesthetically? An interesting choice, if a weird one. But damn, "not even allowed to be born into this world" is definitely a little heavier than "You died"

Silent Hill 4 - I kinda wanted to like this one more than I do, to be honest. The screen effect is cool, the sound is... decent. It just doesn't feel like a full package to me though. The fade out is a little awkward and stilted, the font choice is a little unfortunate because with how small it is on the screen I swear it looks like CAME OVER and that makes it hard to take seriously. FONT CHOICES MAKE THE DIFFERENCE
