Top Tier
Batman: Arkham Asylum
Undertale (get dunked on)
Kirby's Dream Land 3
Sega Rally Championship
Ninja Gaiden - This is one I saw way too many times. Continue screens are already anxiety-riddled by their very nature, but for some reason NG felt the need to go even farther and put a little falling buzzsaw on your character. And the gore of what's implied there would've felt weird in that game, so we get a sudden cut to red with an iconic "ohhhhh" which is legitimately better than if we had actually seen it

Upper Tier
Metal Gear Solid 3 (Time Paradox)
Comix Zone - Honestly, if it was just the first part, with the hand reaching into to cross out the player's dead body, I'd probably have put it top tier. That's creative, its distinct, and it's visually satisfying. The ten seconds or so at the end didn't really click with me the same way, but overall still great.
Bayonetta - Violent, cruel, and still has Bayonetta's stylistic flourish with the flower petals sweeping in and intimidating spotlight at the end
Chrono Trigger (Bad Ending)
Romancing SaGa 3 (Bad Ending)

Mid Tier
Resident Evil 3 - I like what it attempts here. It's weirdly brutal (not gory, just really driving in the "you lost" beating), but it gets a little carried away with what it tries to do, with some silly windows movie maker effects toward the end
Shadow of the Beast
Dark Souls
Donkey Kong Country (just the first one) - Kind of a rehash of what I said before, but this was possibly the best DKC game over screen. Quick, simple, but still brutal and feels bad in the right way.
Waxworks - This was disturbing. And jarring. Nothing about those weird vines led me to expect sudden Splatterhouse levels of gore. Even the game itself doesn't seem to notice the bizarrely upsetting visuals it just showed, with a chiptune theme that sounds like it's out of a silly platformer and just the message "Play again?" as if it didn't just show a guy being slowly dismembered.
Total Distortion
The Punisher - Big points for creativity and for being so riddled with anxiety during that countdown I wanted to press continue for them. Loses points for that heartbeat not matching the music, the countdown, or even the heart monitor on the screen. Everything is so out of sync it borders on headache-inducing
Ultionus: A Tale of Petty Revenge
Castlevania: Symphony of the Night - top tier for that screen transition, bottom tier for that blindingly terrible font choice

Lower Tier
Donkey Kong 64
Theme Park
Donkey Kong Country (the entire series)
Baldur's Gate - I mean, the animation itself isn't bad, but as a death screen, it's slow and drawn out and would likely get old after like 2 times of seeing it
Worms 3D

Bottom Tier
Conker's Bad Fur Day (first death) - I hesitate to even really count this, because it's not so much a death screen as a cutscene, but unlike some of the other "scenes" nominated this one feels like an intended part of the story rather than a game over, but that's not why it lands this low. The humor of Conker's Bad Fur Day, in everything I've seen about the game, just never really seems to click with me at all
