Sticking to national chains... McDonald's did away with the one and only thing I'd ever eat there. I don't remember what it was called but it was bacon and onion strings or something. I now never eat there, because that was the only thing they had that I liked (Wendy's is better for bacon burgers anyway). Culver's also has the pub burgers on a temporary basis, but at least I know they're temporary.

Most of my "usual" places are local restaurants. A brewpub stopped serving flatbread pizzas a decade ago, which sucked for me (even though that place is still awesome). Another stopped with the pizza burger and the thickest, juiciest mozzarella sticks around, and losing those also sucked (that place is also still awesome).
