So you've been having problems since primary/secondary school, specifically in motivation and focusing.. You might have ADHD. There's treatment for that. Stimulants like Adderall can really help. I would talk to a psychiatrist to get a diagnosis on your mental health and get treatment (you also have depression and anxiety, which could be from a multitude of things). It an really make a difference.

The fact that you were eligible for AP Physics, means you are reasonably intelligent. You failing it as well as getting a 2.1 in CC doesn't mean you are stupid. And it sure as hell doesn't mean you should make it become a self fulfilling prophecy. You lack drive and focus, and you don't know what career path to take. But if you keep self pitying yourself and making excuses and do nothing, then you will become nothing. But you have a whole life ahead of you, and you can turn it around now. Just change your mindset and don' be stuck in past failures and not use it as an excuse to hold you back.

For a career, you can look into fields you think you can enjoy doing while offering a decent amount of money and room for growth, and having plenty of job opportunities. Another idea is that you can also think of something that can help change the world for the better. You can also weigh in your strengths and weaknesses and talents as well.

You can never go wrong with STEM fields or business. Healthcare, biotech, computer science (software or hardware programmer), IT, engineer (so many different types), business development, etc. You can also become an entrepreneur.

But you have to put the work in it. It won't be easy, but nothing in life worth fighting is. But its better than staying and going nowhere. Have short and long term goals, do some research on career paths and work through it. You owe it to yourself to not give up on your life. There's ways you can earn money on the side too. Stocks, entrepreneur stuff (even making a youtube channel)
