It's totally plausible, depending on the firearm and condition of it. There's a reason it's not a great idea to load an SKS indoors, as the simple act of letting the bolt close can set off 1-3 rounds from a stuck firing pin. No need to touch the trigger and the safety won't stop it.

That out of the way, I still can't fathom people that try to clean the firearm without checking it first. It's complacency at its finest...
"oh I've handled firearms for years, it's totally fine" *pulls trigger on a loaded weapon, because it became routine*

Happens more often than you think, people like these are what make gun owners look bad. I've seen so many dumb things happen at the range from complete strangers.
