It can only be used once per item.

You can use it on any weapon, armor, helm or shield (magic, rare, set, unique, crafted or white) that doesn't already have sockets. Rare, set, unique and crafted items always get 1 socket. Magic items have a 50/50 chance of 1 or 2 sockets. And white items get the max number of sockets (but this varies according to ilvl, as you can see in this table: https://diablo2.diablowiki.net/Sockets#Sockets_by_Number).
