it depends a little on the game and settings. For OoT the big things are that you can randomize the exits and/or what's in chests. So you could have kokiri forest lead into zora's domain, and have all sorts of items end up in weird spots.

You'll probably get stuck for quite a while at a few different points going back and checking everything until you find out (for example) a random chest in a hole in hyrule field has the ocarina of time in it

Randomizers have logic systems to make it so that the game isn't unplayable. You can usually tell it to make sure the game is winnable without sequence breaks. Also a lot of them will make a spoiler file for you that tells you where everything is that you can use if you get hopelessly stuck

They do tend to take a pretty long time if you don't know the original game really well
