I've played tons of Atlus games and never felt as bogged down as playing DQXI Dragon Quest is all about your characters on a journey, with stops that may or may not immediately advance the overarching narrative. So yes, you might have to spend time in a town solving their town-specific mystery. In this way, it's a lot like (early) One Piece. Is "Sky Island" filler? Some people say yes, some say no. Having a "loose" story rather than a tight narrative like Persona 5 is just a different approach to storytelling. I think it's calming but people are fair to say they find it boring. In some cases this approach has worked out poorly (like DQVII - which the above poster mentioned). If you think nothing happens in DQXI, just try playing that one.
