I think Cal can answer better for the later ones but concerning the first three I believe its the same world, just different times. Anime wise, GX takes place within the lifetime of the characters from Yugioh. 5D's is the future of those two, but distant enough from either that they needed ~~time travel~~ to bring all three main characters together.
