I think my biggest pet peeve is when I sign my name in an email or something, and the reply is "dear [misspelled name]"
People do that with my first and last names, even though my last name is literally a common, basic, first-grade reading word. My FirstName.LastName is my email and I always sign it. My first name is a little long, but it's a common enough name on its own and its elements are made up of very, very, very common names. And I pretty consistently get back the equivalent of "Dear Firstnam Lastnamer."

Nightmare scenario that happened: Once, I was e-mailing with someone who had a non-typical name for work and I was really, really, really trying to get it right, so I copy/pasted her name out of the e-mail (FirstName.LastName format and I used just the first name). Then I looked at how she signed her name and she had spelled it differently than in the e-mail. It wasn't something that could have been an accidental typo while she was signing like 'a'/'s'; it was something like an 'e' where there had been an 'i.' I felt like crap and she never responded, so I don't have any data points on the actual correct spelling.
