Likely a defective sebaceous cyst that secretes oil between the tissues instead of onto the skin. They usually marsupialize (get so defective the cavity directly heals onto the edge of the skin, creating a stable pocket instead of a growing ball), but somtimes they just keep growing or get infected.

Would be exceedingly rare to be a cancer. The main risk is that it can be a source of gangrene of your scrotum if you were ever to need long term steroids, develop bad diabetes, or the like.

If it grows your PCP or a surgeon should lance it or remove it.
