A unified healthcare system would help reduce instances of this. I don't mean some sort of Medicare for all system, just a system where medical records from another facility are easily accessible by new doctors at a different facility. The new doctors could look and see what sort of claims have been made to previous doctors and see what sort of procedures have already been done. They just have to look up the patient in the unified system and can see their entire history. Seeing their full history would make these sort of cases easier to spot, often before the parent is able to cause permanent harm to the child.

The problem is, who would hold that data? A lot of people wouldn't trust the government to have that kind of data, and I would imagine that even fewer people would trust a private company.
