Looks like it is a flat 1-1.5% if the opponent is at 20% or lower and goes up linearly from there. Death is guaranteed at around 160%. This assumes that Hero is at 0%. Testing for Hero at higher percentages still needs to be done. I'm actually a bit surprised this testing wasn't done before. --- Link isn't the only one who kicks ass.
