TopicI never had a system die catastrophically but I did have work equipment do so.

Back when I was beautiful, I worked at a Little Caesar's during the morning hours which involved making dough. A daily job we were tasked with which involved making a mixture from water and packets dumped into a massive cheese mixer with flour. Yes, the dough at Little Caesar's IS fresh and made daily.

Anyways, the cheese mixer at my Little Caesar's was older than I was (I was about 25 at the time) and over time, it became increasingly troublesome, making strange noises, the lid breaking off, the switch to turn it on requiring someone to hold it while it mixed. The reason why we never replaced it during this time was because these things are expensive.

Eventually, during one day during a hot summer, the mixer sounded especially grouchy and after turning it on and holding the switch, a shower of sparks exploded from the bottom of it. We didn't get much dough done during that day and eventually, got a newer model. What happened was probably the ball bearings came out and got trapped in the engine of the ancient thing.
