If the VP and a majority of the cabinet (technically whatever group Congress decides, but it is generally assumed to mean the cabinet) declare the President is unable to serve for whatever reason he is removed and the VP takes over. (The President can tell Congress he is able to serve and it gets a bit more complicated if that happens and it's still contested by the VP + Cabinet but yeah) An amendment isnt in the constitution and that amendment is about declaring him unable to serve.
