You either start a quest from the overseer's office or you stumble across one while a dweller is exploring the wasteland. The locations are built the same way as the vault and all the rooms start off blacked out until you go into one. You explore looking for whatever the objective is, killing enemies and searching containers along the way. Some even have selectable dialog like the main games. You still have to wait for a set amount of real world time to travel to the quest but it can be sped up with nuka cola that you can buy with real money but they're easy enough to earn without buying.
