Uh, not entirely sure what the question is, but the vast majority of self defense courses teach you to carry a firearm with a full magazine, and one in the chamber. If you carry a gun with a safety, it's usually switched off mid-draw, so that when the gun is on someone, the person needs only pull the trigger.

And revolvers often just have a very heavy trigger, with strong springs to make the amount of force needed to fire off a round a very deliberate, conscious decision.
