Are we talking strictly about the Western empire , which formally died c. AD 476, or the Eastern Empire, which technically hung on til 1453?

I bet there's probably a few other institutions/governments that have survived in some form since 1453. I guess it also depends on what you would define as an "institution".

No, I mean the western empire. If we talk about the eastern empire technically you could say it continued even later than that with the Ottoman state.

The Ottoman empire was not Roman at all. The Eastern/Byzantine Empire was literally a Roman remnant, the Ottoman Empire was an entirely new state on the same territory.
