Like "I observe this patient has a penis and testicles, so, because those are male genitals, this patient id almost certainly male." no dammit, you're supposed to sit down and interview every patient and confirm what they identify as. and you need to make sure that they're always identifying as that thing, and that any treatments that presume an identification are run by them first. --- These people are literally ignorant of the law.
