My sophomore year, my chem teacher had us fill out this form on the first day of class.
At the very top it said to make sure you read the entire document.
The first couple of questions were standard: name, grade, age.
But the next like 7 were activities, like doing 10 jumping jacks, measuring how many steps it takes to go from one side of the room to the other heel-to-toe, other random ****.
The very last question was "Skip questions 4-10 (or however many) and sign the here showing that you have read the entire document."
He got a LOT of people with that ****.
