It's simple, but it gets the job done. I've given each system its own tab, and each game has the following informational columns:

Own - Do I own it (obviously, yes for all)
Played - Have I played it
Beaten - Have I beaten it
Year Acquired - The year (or approx. year) I got it
Year Released - Release year for that system (so FFVI/III on Wii is a 2011 release)
Developer - Who developed it
Publisher - Who published it
My Rating - My own simple rating scale (A-F) only for games I've played enough to justify one


Clicking on any game opens a link to its Wikipedia page in case I'm interested in some more information. The "Totals" tab at the end just lists the combined totals of games per system, my personal ratings, and the release years.

I'm thinking of expanding it by including "Totals" columns for Developers and Publishers, and maybe some other ideas as well. Tell me what you guys think, especially if you have some ideas or improvements. If you like it, feel free to modify it for your own collections - it doesn't take as much time as you think since all the formulas are already in place.
