The lion's share goes to hospital administrators and doctors. Because hospital prices are so high, insurance companies are "justified" to have high premiums and deductibles... which means the insurance companies milk the consumers, then pay kickbacks (which are made of other people's money) to the hospitals to keep the incentives up.
