Out of network for me is covered if an in network doctor makes the referral. And even if I do the whole thing out of network like I have to go to an ER and don't get a choice where, the OOP max still applies. The only thing it doesn't apply to are things that are not part of my policy.
Never heard of a policy that allowed the OOP max to cover out of network providers, the premiums must be insane.
