There's different teams working on all chapters at the same time. Now imagine if half of those teams quit or moved to another project in the 10 years of development. Then the other half had to redo everything in the 2 years after Nomura was fired. It ends up with a lot of cool gameplay and story elements, but definitely no throughline.
