The case for Electronic Gaming Monthly:

When Final Fantasy VII came out I pretty much bought every magazine that featured it on its cover. Here was EGM's:

I remember this art also being in the unofficial player's guide for the game, which was MUCH better than the Brady official guide (which spoiled everything for everyone and also wasn't particularly helpful!). Look at these terrible other games on the cover though: Spawn and Mission: Impossible! The Bruce Willis Neversoft game! (Darkstalkers 3 rules).

A few months later, EGM followed this up with this square-centric "masterpiece":

It belongs in a museum.

However, the issue of EGM that stands out most in my mind is this one with a South Park (N64) cover.

This is such a terrible cover from a visual design standpoint. It's not even the one I had: I had one with Cartman on the cover. It was still cluttered as all get out though. I believe they reviewed both MGS and OoT in this issue and they both got perfect scores. Or maybe one person gave OoT a 9.5 or something, which was somewhat controversial.
