More than likely someone in the medical billing department fucked something up or their attempting to balance bill you and most insurance contracts don't allow that. Also check your evidence of coverage and see what it says about the ER. Sometimes there are clauses in those that wont cover stuff if it's not life threatening. This Medical billing is often fucked up all over the place.
