Ask them for an itemized bill, and then ask your insurance for an EOB (explanation of Benefits). Then compare those. If you need to get their billing department on the same line as you and Aetna's claims department.

More than likely someone in the medical billing department fucked something up or their attempting to balance bill you and most insurance contracts don't allow that.

Also check your evidence of coverage and see what it says about the ER. Sometimes there are clauses in those that wont cover stuff if it's not life threatening.
