I did knife only trophy on hard mode. Can't remember if you can use defense item's for the trophy. If not you have to run in circles and take advantage of the autoaim. The dog will only one hit Ko you if you are low on health. Spam the buttons to break free.

You can't lol. I'm playing on very easy tbh <_< I gotta respect doing this on hard with little healing.

It's going a lot like the speedrun tbh, which makes sense since the routes are really good about avoiding enemies. I know there's a lot less enemies in general which really helps. For instance, there's only one dog when you blow the whistle of very easy and it so took me 4 tries. Supposedly crimson head in the grave is the hardest boss knife only.
