It's ultimately NBC's fault for trying to have their cake and eat it too. They didn't want a repeat of the Letterman situation, so they tried to create an orderly succession from Leno to Conan. But they also didn't want to lose Leno or Conan to another network and have to give up another slice of the late-night pie.

So in the end, they created a situation that worked for nobody. Leno was unhappy at 10 PM and the show did terribly. Conan was undercut by having his predecessor kicking around and serving as an awful lead-in. And then NBC worsened the situation by trying to walk-back their decision after less than a year, rightfully angering Conan who felt the network didn't have his back (for comparison's sake, Leno did poorly against Letterman for nearly two years before he hit his stride).

The smart move would've been to either cut Leno or Conan in 2009 and put the full network weight behind whoever remained.
