For me, finding a sense of purpose turned everything around. It was difficult as hell to do that though because a combination of genetics and trauma turned me off to the normal things that motivate people. Lifelong morbid obesity and later alcoholism destroyed what little ambition I had.

I ended up defining my life by the things I didn't want (reproducing, wealth, prestige, fame, excellence, etc) and that just caused me to stagnate. I got to a point where it was either dying or finding a reason to live and in that desperation, I managed to find some things that motivate me. And I found them in places and through people I'd never imagined, which is something I think a lot of people are prevented from experiencing.

You get this pretty narrow idea of what's an acceptable goal in life, what's an acceptable reason for having it, and what's an acceptable path to get there. If you don't/can't fit that, then nothing else really matters. Tacking on mental health and other obstacles, it's pretty easy to just not care and not act.
