In the comic the conflict was about whether or not super heroes should be allowed to keep secret identities. A new law gets passed that every super hero has to register with the federal government and let them know their real identity. It still gets kept from the average Joe but the government knows who every hero is

Also Tony ends up arresting a bunch of unregistered heroes and literally locks them up in a prison that is located in another dimension on some barren planet. He was cartoonishly wrong throughout the story.
I haven't read the comics, but I do know that Spider-Man joins Tony's side on this one. I don't know if he chose to publicly join his side and announced his identity or if the whole "Only the gov knows who they are, it's not released" is wrong or if someone in government hated him, but Peter starts being hunted down. He pretty much has to go into hiding. Eventually, someone finds him and kills Aunt May.
Obviously, Peter's devastated by this. He realizes that forcing everyone to reveal their identities isn't good. He finds Tony, sticks him to the wall, and yells at him.
Then later he goes and talks to Mephisto and sacrifices his marriage with Mary Jane in order to get Aunt May back but that's all a separate thing.
