Here's how it actually kills you since a lot of people still think this is like the Flu or Tuberculosis.

When you get infected, it causes an immune response to have your white blood cells swarm your alveoli. This makes less room for red blood cells which means you're not getting oxygen even if you're breathing.

All the other shit like headaches and fever don't really matter here. What kills you is the lack of oxygen. You're suffocating while otherwise breathing normally.

People who are overweight are at risk because they probably also have sleep apnea which further reduces oxygen while sleeping. People who smoke, same deal. They're already getting less oxygen.
