The hardest ones I remember were for opening every shiv door in one playthrough and getting to the end of Factions as both hunters and fireflies.

The former just requires careful planning, and the latter just requires that you know how it works and can tag three enemies on the last day.

There were some pretty BS multiplayer trophies but at least in the PS3 version the only ones required for the plat were those two I mentioned above.

I don't really remember if there was one for Survivor mode but Survivor mode wasn't anywhere near as hard as S-Ranking Revengeance mode.
