1) I'm pretty sure this used to be a thing, but it just meant that the queue went a lot slower because it adds up when you have to add notes for every moderation. I personally try to add notes for anything that might not be explicitly clear to the end user.

2) It staying on history is a benefit to you in the end. It's not held against you in any way.

3) Yeah, that would be a great way to set up volunteer mods for harassment. This idea sounds more and more ludicrous every time I see it proposed. A moderation could be blatantly fair but there are people who will take it out on the mod they see modded them.
