You know, I have always been curious how scenes like that are filmed. Like you have this woman standing topless in front of a kid in a film set. I can imagine the kind of coaching involved a) for the kid to keep a straight face through all the takes b) for the woman to keep the movie pg and c) for the kid's parents who are likely on set watching it happen.

It's one thing to have nudity in movies during sex scenes and scenes with adults, but it must be a whole different ball game when there are children involved. Even if the audience doesn't see the nudity since it is censored, there is still nudity on set that must be handled tactfully.

The animated scene from kill bill vol 1 happened because the MPAA was going to give it NC17 if he used a real actress for o-ren ishi.

So it's definitely a concern today. I'd argue it's probably progress since the reasons those scenes above got shot is people back then just didn't take female on male rape seriously.
