I worked in fast food for too many years, and it was very common for people to never look at the menu until the moment I ask "how can I help you?". This includes when there is a line and they had multiple minutes to decide what they want before coming to the counter.

People would rather ask me questions about things easily seen on the menu then to look at the menu beforehand. The main question being "how much is..." when every price is right there on the menu. I literally could not go 10 minutes without having to tell someone a price. I memorized most items price (for exactly this reason, I have to give a price on every other customer) but I loved it when I didn't know the price because it was a new item or the price changed, so I would get to turn 180 and look at the menu and read it for them, maybe even point to the menu board as I read it. And for new items they usually get their own menu board, complete with a giant picture of the item and the only text being the name and the price, and they still can't figure that out.

When lines are held up, usually it's from people asking questions on things they can easily find themselves.
