Well in the OoT Item Randomizer, which is designed so every seed can be beaten completely glitchless, the Kokiri Sword is only required for one thing. Child fishing.

For whatever reason if you go into fishing with a blank B icon you can't cast the rod, even though you can fire the Slingshot in the Market Shooting Gallery and drop Chus in Bombchu Bowling with a blank B icon, so you need the sword equipped to get the prize from child fishing. Otherwise everything else can be done with Deku Sticks or other items.

So assuming you play with the Open Forest setting (which eliminates the need to show the Sword and Shield to Mido; pretty much everyone plays with this setting) the Kokiri Sword can be placed literally anywhere in the game by the randomizer except child fishing.
