I was stuck at a light turning left the other day for about 20 minutes because the cross traffic kept obstructing the intersection even though it was clear that they were going to be stuck there during the cycle. NYC has a clearly defined box for every intersection and if you are in the box when the light turns red you'd get a ticket from a traffic cop. People don't respect the box anywhere else, because they feel that they "deserve" to go, even when they know they'll block everyone else.
