There are several things of note.

One is the concept of a chat room. Remember chat rooms? They were big back in the late 90's. Lots of people were into those. Twitch streams are essentially chat rooms with a host. There's also a videogame aspect. Watching someone play, talking about the game and games in general. A lot of people played games with friends or siblings growing up. It's not uncommon at all. The chat room aspect plays into that. Also, the streamers that tend to have a loyal following are at the very least somewhat charismatic.

So if someone can't understand the appeal of watching a twitch stream, perhaps they could understand the appeal of a chat room and/or the appeal of playing games with someone else. It's really not that illogical of a jump as far as entertainment goes.

Huge streamers are a different story, as those chats are a mess but I assume the hosts are particularly charismatic. Probably a big part of it is all their friends also watch. I would theorize that the mega streamers have a younger audience on average.
