Which episode are you referring to?

Anyway, It's been stated repeatedly in the reboot that The Doctor is the name he chose for himself. In season 10 Missy flat-out states his name is Doctor Who, but this is almost certainly a lie. The point is, the show is deliberately and consistently ambiguous about his name.

Then in "Twice Upon a Time", they had the First Doctor joke about calling himself Doctor Who because he'd tell people "I'm The Doctor" and they'd say "Doctor who?".

Which is actually a callback to the show's very first episode, when Ian confronted The Doctor; only knowing him as Susan's grandfather, he called him "Doctor Foreman", to which The Doctor replied "Doctor who?"
