It's not a coincidence that the Shogunworld copies Westworld, that's the point. The writers have hammered it in our faces that the company is all about the immortality stuff and that the park is a low priority. This is just another example of where the park cuts costs, by repeating many characters all over the place.
Specifically, the scene it recreates is from when Sizemore has to rush to change things because of Ford.

Yeah, it still feels very lazy.

Not every host is malfunctioning. Some hosts have changed such as Maeve and Doloros, others are remembering their "other" lives, but the majority are going about their lives as if they were real (not all on their loops, depending on how often they reset). Shogunworld is not unique in that regard, it's just that everything started in Westworld so it has the worst effects.

This is where the inconsistencies come. They really don't show the majority of hosts staying on their loops, they show most hosts going around and killing people indiscriminately. You could argue this happens the most in West World because of Doloros, but that doesn't explain why the hosts in The Raj went rogue. But if they went rogue it implies that all hosts are going off, but in Shogun World, they are apparently still following their loops for some reason outside the bad guy who is malfunctioning and the other two because they apparently meet Maeve. .

The guests were killed by the hosts, we saw that as they were being walked into the town at the beginning. Remember, Shogunworld is considered to be the most hardcore park, so it's easy to believe the guests who were there failed to escape.

Where did we see that? The only people shown dead were Security guys in the forest after that scene, in the scene where they walk into the town there are absolutely no guests there dead OR alive.

Maeve killing the people was a life and death situation every time. Until she learned to control it, it was always a reaction and not a controlled action. Easy to accept why it went the way they did.

Except the second she learned to control it she literally had an entire army off themselves off screen so....
