Just digging right into the fat and organs, ripping them out and eating them. In terms of pure grotesqueness, that'd be it, but I also saw a video of a guy getting castrated in his room which bothered me more. No idea what the circumstances were - it definitely wasn't an operating room, and the person doing it definitely wasn't wearing proper surgical attire or even used proper tools.
