More than anything, it pushes the medium into uncharted territory, and it does all of this in a short timeframe. It's not perfect -- the actual bullet hell gameplay can get dull, but even then the game sometimes finds ways to turn it into something unique that I haven't seen before. I can only think of one game that made me laugh until I cried, made me fear enough to want to cry again, and made me feel so deeply that I cried again and again. I lost a lot of water playing that game. EDIT: Spoiler-tagged things because I always fear overhyping this game to people who have never played it. --- E come vivo?
